The Orbiting Astronomical Observatory (OAO) launch was threatened by the transistor failures encountered during simulated Space Testing. Investigation identified High Voltage breakcown as the culprit.  This paper was the first reference to "Outgassing Phenomena" associated with epoxy encapsulation of cord wood assembly. The final solution was to move the High Voltage elements to a separate module. OAO operated for many years
